Sibling Discounts - A sibling discount of $10.00 off each additional registration applies to all immediate family members with a maximum fee of $400.00 per family. (Sibling discount does not apply to online registration fees.)
A 'Fit to Play' health physical is required to participate in both the football and cheer program, with the exception of the 4 year old cheer mascots. Participants will be required to provide a fit to play physical at their own expense and provide the league with a copy of a physician's release before the participant may practice. This is to ensure the safety of participants and is strictly enforced. We will have exams on July 23, 2011 at 8am and they will be $15.00 per child.
We will need a copy of a certified state birth certificate for each registered participant. Please bring a COPY to registration. All coaches are required to have a copy of each participants birth certificate on file at games. Per NAYF rules, no participant (football or cheer) will be allowed to participate in any PVYFCA activity for the ENTIRE season if a copy of their birth certificate and "Fit to Play" are not on file with PVYFCA by August 17, 2011.
